Schedule of Services and Payment Policy

 

 

Psycho-Educational Assessment Options:

• IQ Test Only - typically needed for admittance to gifted programs 

• Comprehensive Cognitive Evaluation

• Comprehensive Cognitive and Academic Evaluation

• Comprehensive Cognitive and Social-Emotional Evaluation

• Comprehensive Cognitive, Academic, and Social-Emotional Evaluation

• Social-Emotional Evaluation (including AD/HD screening)

 

Except for IQ Test Only Assessments, the following deliverables are included:

• A 1-hour, face-to-face initial intake appointment with me;

• One or more testing sessions with your child as reasonably necessary to complete the assessment;

• Scoring and interpretation of protocols;

• A written report detailing testing results and recommendations; and

• A 1-hour, face-to-face meeting with me to go over the report.  

    

    *50% of the assessment fees are payable before testing of your child is conducted.  The balance is due before

      the report is delivered to you.  Reports will not be released without full payment.

 

Educationally-Related Counseling:

All counseling services are billed at $125 per 50-minute session.  Fees are payable prior to each session.

 

Payment Policy:

All fees are payable by cash, check, or money order only.  I do not bill insurance companies but am happy to provide a receipt delineating my services and the associated costs.

 

24-Hour Cancellation Policy:

Clients are required to provide 24-hour notice of cancellation to avoid being charged $50 for a missed appointment.  This fee helps me cover office rental costs that I incurred when you confirmed the original appointment.
